Forms needed for participation in school sports for the 2012-2013 school year:
1. IHSAA Physical (must be dated after April 1st, 2012)
2. Athlete Authorization Form
3. Concussion Acknowledgement Form
4. Emergency Medical Info Form
All of these forms can be found under the "documents and forms" tab on our athletic website. Forms must be turned in prior to participating. This includes any summer practices/open gyms. Parents are encouraged to make copies of these documents prior to submitting them to the school.
