All Athletes Must Have a Physical On file In Order to Try out For a Sport

IMPORTANT INFO REGARDING IHSAA SPORTS PHYSICALS - PLEASE READ ALL!!!

Any  7th, or 8th grade student who wants to participate in any school sport during the 2016-2017 school year MUST have an IHSAA sports physical on file with the Athletic Director before participating. Physicals may be turned into the athletics office or to the coach on the first day of tryouts. The physical must be dated on or after April 1st, 2016. The most updated IHSAA form will be the only form accepted. The form can be found on our athletics webpage.

*The form must be signed by a physician (MD or DO) on page 2. No nurse practitioner or physician's assistant signatures allowed.

*The physician's license # must be included on page 2. 

*NO stamped signatures will be accepted.

*Student-athlete & parent must sign bottom of page 1 and page 4. 

*DO NOT EXCLUDE ANY PAGES OF THE PHYSICAL!

A Message From The Athletics Department

The Charger athletics program is an integral part of the total Junior High School program and thus reflects the school's overall commitment  to excellence. We are proud to provide a number of athletic offerings for our young people. We believe that providing these opportunities for competition enhances the overall development of our student athletes and extends to the educational mission of our school.

If you believe that organized sports can contribute to living a healthy life, that the positive values and virtues developed on the playing field can last a lifetime, and that good character influences enjoyment, satisfaction, and performance, then our athletics program is for you. This program provides opportunities to teach values such as sportsmanship, honesty, dedication, teamwork, self discipline, loyalty, and cooperation. The teaching of these values is essential to the education of our youth.
