New Immunizations Required to Enter Sixth Grade

The state has changed immunization requirements for entry into sixth grade. As a result, sixth grade students cannot attend school unless they can show proof they have had the following immunizations:

3 Hepatitis B
5 DTaP
4 Polio
2 Varicella (Chickenpox)
1 Tdap (Tetanus & Pertussis)
1 MCV (Meningococcal)

HSE nurses have updated information they have and then contacted parents of students whose immunization records are incomplete. To meet new requirements, we recommend you go to your child’s health care provider or the health department to obtain the needed shots as soon as possible. Because those offices are extremely busy at this time of year, you will need to act quickly to get an appointment. Once you obtain your child’s updated information, please fax or mail your child’s immunization record to his/her school to the attention of the school nurse.
