Immunization Clinic Information

Immunization Clinics at Hamilton Southeastern Schools:

As a community service, Visiting Nurse Service at St. Francis will be coming to HSE Schools on Thursday, August 11, 2011, from 1:00PM-4:00PM at the Sand Creek Intermediate School to administer Indiana school-required vaccinations (Meningitis, Tdap and Varicella/chicken pox) to students 6th through 12th grades that request to be vaccinated with parental consent.

Please contact your health provider if you are unsure if your child(ren) need these immunizations.

If you have a question about your child’s health conditions and wish to discuss if they should be vaccinated, please call (317) 722-8299, extension 1296, for a nurse.

It is NOT mandatory to have this done by Visiting Nurse Service, Inc/VNS. This clinic is being offered as a convenience to parents. These are the same FDA-approved vaccines that you would receive at your doctor’s office.

Please complete the attached Consent Form. Cash or checks will be accepted. Please make checks payable to "VNS".

Costs:
Medicaid A and C: No cost
All other students: $14.47 flat rate for one or more immunization(s)
