Registration Forms and Information
Listed below are registration forms for Fishers
High School. they are organized into required,
optional and informational groups.
Many of the documents were included in the summer
registration packet mailing. If
you did not receive a registration packet, please
call 317-915-4290, and we will send a packet to you. You may also stop
by the main office or guidance office to pick up
the registration documents after August 1st.
NEW STUDENTS WISHING TO ENROLL AT FISHERS HS
Students wishing to enroll at Fishers High School should refer to the HSE
Enrollment Information and Procedures. It is important to call the Fishers High
School Guidance Office at 317-915-4293 to set up an appointment for enrollment.
REQUIRED DOCUMENTS AND FORMS
Everyone MUST bring the following completed forms
to registration. These required
forms were included
in your registration packet and must be returned
at registration. These forms are not available online:
- Attendance
Signature Card - available in the attendance or main
office (everyone)
- Health Card - available in the
clinic (everyone)
